ABSTRACT

Introduction: The cecum is the first part of the large bowel. Cecectomy is a sufficient treatment for some patients, avoiding overtreatment by ileocolic resection. Purpose:The goal of this study was to review a surgeon's experience with laparoscopic cecectomy and provide a technical video demonstration of this uncommon operation. Methods: A retrospective chart review was conducted of all consecutive patients treated with laparoscopic cecectomy over a 16-year period. All operations were performed using a 3-trocar technique. The cecum was transected with 1 to 2 firings of a 60 millimeters linear stapler, preserving the ileocecal valve and ascending colon. Results: 19 patients were identified including 12 females (63.2%). Median age was 42 years (range 16-84). Indication for surgery included appendiceal pathology in 12 patients (63.2%) and cecal abnormality in 7 (38.9%). There was no conversion to open surgery. Median intraoperative blood loss was 25 ml (range 0-150 ml) and no patient received a blood transfusion. No intraoperative or postoperative complication was noted. The median length stay was 1 day (range 0-6). Readmission rate was 0%. Final appendiceal histopathology revealed acute/chronic appendicitis in 5 patients, mucinous cystadenoma in 4 patients. Cecal histopathology revealed adenoma in 4 patients. Median follow-up was 16 months (range 4-53). Conclusions: Laparoscopic cecectomy is a sufficient treatment for some patients with benign conditions of the appendix and cecum. It carries minimal morbidity. It should be considered as an alternative to segmental bowel resection in a select group of patients. (AU)

BACKGROUND: The treatment of Crohn's disease (CD) is based on medical therapy and surgery is reserved for failure of medical management or complications. AIM: To evaluate endoscopic, clinical, and surgical recurrence of CD after surgery. MATERIAL AND METHODS: In a prospectively maintained database, consecutive patients older than 15 years subjected to an ileocecal resection for ileocolic disease from January 2011 to April 2021, were identified. The diagnosis of CD was confirmed with the pathologic report. Patients with less than one year of follow-up were excluded. Information was obtained retrospectively from the database and clinical records. RESULTS: Fourteen patients were identified. The mean age at the time of surgery was 38 years. Surgery was performed at a median of 41.5 months (0-300) after the diagnosis of CD, nine elective and five emergency procedures. In five patients there were four major and two minor postoperative complications, with no anastomotic leakage. Six patients had endoscopic recurrence and seven had clinical recurrence (50%) at a mean of 15 months, one of whom required a second operation. There was no mortality. CONCLUSIONS: After the surgical treatment of CD, the clinical and endoscopic recurrence rate continues to be high.

ABSTRACT

Uma mula hígida, de cinco anos de idade, foi atendida portando um aumento de volume na região abdominal lateral direita, estendendo-se da região inguinal até 25cm cranial à prega pré-crural, de 60 dias de evolução, após trauma com um touro. O volume era redutível e sem dor à palpação, com presença de intestino grosso envolvido por área circular hiperecogênica na imagem ultrasonográfica. O diagnóstico foi hérnia inguinal, e a paciente foi submetida à anestesia geral inalatória para abordagem cirúrgica pela região inguinal, onde se visualizou o ceco emergindo pelo anel inguinal externo, cujo diâmetro era de aproximadamente 10cm. O ceco apresentava coloração e conteúdo normais e parte do corpo e o ápice projetavam-se cranialmente para uma bolsa de tecido subcutâneo, que foi aberta para facilitar o reposicionamento do órgão ao abdômen, bem como para ampliar o anel inguinal externo. A paciente teve alta 18 dias após o internamento. Com base neste relato, é possível concluir que a hérnia inguinal indireta não encarcerada pode se desenvolver em fêmeas equídeas após trauma. De acordo com a literatura consultada, o presente estudo é a primeira descrição desse tipo de hérnia em uma fêmea equídea e a primeira envolvendo o ceco.(AU)


A five-year-old healthy mule was referred with enlarged volume in the ventral right side of the abdomen, with 25cm in length from the inguinal region to precrural fold, developed 60 days previously due to a traumatic confrontation with a bull. The mass had no pain and was reductible on palpation and the ultrasonographic examination revealed large intestine surrounded by a hyperecoic ring. Based on these findings, the diagnosis was non incarcerated inguinal hernia. Under general anesthesia, the patient was submitted to surgery by direct approach on the inguinal region, when the cecum was seen emerging from a 10cm overture of the external inguinal ring and projecting to a subcutaneous fold cranially to the ring. This fold and the external inguinal ring were opened to facilitate the reposition of the cecum back to the abdomen. Eighteen days after the surgery the patient was discharged with a satisfactory recovery. Based on this report, it is possible to conclude that females can develop indirect non incarcerated inguinal hernias with the involvement of the cecum with good prognosis. Based on the consulted literature, this is the first report of this type of hernia in a female equine.(AU)

ABSTRACT

Ogilvie syndrome, or Acute Colonic Pseudo-Obstruction (ACPO) is characterized by colonic distension in the absence of mechanical obstruction. In general, it evolves favorably following a conservative treatment, and surgical procedures are not necessary6. We describe a case of ACPO with evolution of two days, in a 79-year old male patient, with asthma, type 2 diabetes mellitus, systemic arterial hypertension and policystic kidneys. After failure of conservative treatment based on support measures and neostigmine, percutaneous endoscopic support cecostomy, using a gastrostomy tube. The technique and its early execution were chosen considering that it was easy to be implemented, low cost and need for immediate colonic decompression due to high risk of ischemia and perforation of the colon, associated to a rapid clinical deterioration of the patient


El síndrome de Ogilvie o Pseudoobstrucción colónica aguda (ACPO) se caracteriza por la distensión del colon en ausencia de obstrucción mecánica. En general, el tratamiento conservador es favorable, no siendo necesaria una intervención quirúrgica. Describimos el caso de una ACPO con dos días de evolución, en paciente masculino de 79 años, asmático, portador de diabetes mellitus tipo 2, hipertensión arterial sistémica y riñones poliquísticos. Después del fracaso del tratamiento conservador con medidas de soporte y neostigmina, se optó por la realización de cecostomía endoscópica percutánea de protección, utilizando una sonda de gastrostomía. La elección de la técnica y su realización precoz se dio teniendo en vista su facilidad de ejecución, su bajo costo y la necesidad de descompresión colónica inmediata por el elevado riesgo de isquemia y perforación del colon, asociado al rápido empeoramiento clínico del paciente.

ABSTRACT

ABSTRACT PURPOSE: To investigate the potential efficacy of beractant (Survanta(r)) and Seprafilm(r) on the prevention of postoperative adhesions. METHODS: Forty Wistar-albino female rats were used. The rats were randomly allocated into four groups of 10 rats each as control group (CG), beractant group (BG), Seprafilm(r) group (SG), and combined group (COG). All rats underwent cecal abrasion via midline laparotomy. Before abdominal closure, isotonic saline, beractant, Seprafilm, and combined agents were intraperitoneally administered. Adhesions were classified macroscopically with Canbaz Scoring System on postoperative day 10. Ceacum was resected for histopathological assessment. RESULTS: Macroscopic adhesion scores were significantly lower in BG, SG, and COG than CG (p<0.05); (45%, 15%, 25%, and 15%; respectively). Histopathological assessment revealed a reduced inflammation and fibrosis score in the study groups than CG (p<0.05). In BG, adhesion development, inflammation and fibrosis scores were lower than SG; however, it was not statistically significant. CONCLUSIONS: Intra-abdominal application of beractant is significantly effective for the prevention of adhesion formation with no adverse effect by covering the whole peritoneal mesothelium with excellent gliding properties in a rat model. The combination of both agents is also effective in reducing adhesion formation, however, not superior to single beractant application.

ABSTRACT

ABSTRACT PURPOSE: To evaluate the effect of Schinus terebinthifolius Raddi (aroeira) and Orbignya phalerata Mart. (babassu) in the healing process of cecorrhaphy in rats. METHODS : Fifty four rats were used, distributed into three groups randomly: aroeira, babassu and control, which were divided into three subgroups (six animals) according to the time of the deaths (7, 14, 21 days). All underwent the same surgical procedure, cecotomy and cecorrhaphy. The animals in group aroeira and babassu received daily dose of 100 mg/kg of hydroalcoholic extract and 50 mg/kg of aquous extract respectively, by gavage. The control group received only saline solution. The parameters evaluated were: macroscopic changes, ,resistance test to air insufflations and histological changes. RESULTS : All animals showed good healing without infection. All groups presented adhesions between cecum and neighboring organs. The resistance test insufflating of atmospheric air showed progressive increase of pressure according to the days in the aroeira group, and decrease in babassu group, without significant difference. Microscopy showed significant difference in the polymorphonuclear, hyperemia, angiogenesis, fibroblast proliferation and collagen histological variables in the 14th day. CONCLUSION : Hydroalcoholic extract of aroeira and the aqueous extract of babassu favored the healing process in cecorrhaphy in rats.

PURPOSE: To evaluate the effects of copaiba oil as a prophylactic and/or therapeutic substance on survival of rats subjected to cecal ligation and puncture, describing histopathological and oxidative stress findings.METHODS:Forty rats (Ratus norvegicus) were distributed into five study groups (N=8): Sham group (ShG): normal standard animals; Sepse group (SepG): submitted a cecal ligation and puncture (CLP); Pre group (PreG): administered copaiba oil once daily by subcutaneous injection for five days before carrying out CLP; Post CLP group (PostG): administered copaiba oil once daily by subcutaneous injection from the first day of CLP until death by sepsis; and Pre/Post group (Pre/PostG): administered copaiba oil once daily by subcutaneous injection for five days before carrying out CLP and from the first day of CLP until de death by sepsis. After the death of the animals, blood was collected for assessment of oxidative stress and histological analysis were performed. The Kaplan-Meier curves of surviving time were realized.RESULTS: Survival analysis demonstrated that animals treated with copaiba oil prior to the execution of the CLP (PreG and Pre/Post groups) had longer survival compared to the sepsis group (p<0.0001) whereas animals receiving copaiba only after the completion of CLP (PostG) showed no statistically significant difference compared to the sepsis group. However, when comparing the two groups in which was administered copaiba previously (PreG and Pre/PostG groups), there was no statistical significance between the groups (p=0.4672). There was no statistical difference between histopathological findings or the levels of oxidative stress.CONCLUSION: Prophylactic subcutaneous administration of copaiba increases survival of rats subjected to severe sepsis by cecal ligation and puncture.

ABSTRACT

Anastomotic dehiscence is the most severe complication of colorectal surgery. Metalloproteinases (MMPs) and interleukins (ILs) can be used to analyze the healing process of anastomosis. To evaluate the effects of bromopride on MMP and cytokine gene expression in left colonic anastomoses in rats with or without induced abdominal sepsis, 80 rats were divided into two groups for euthanasia on the third or seventh postoperative day (POD). They were then divided into subgroups of 20 rats for sepsis induction or not, and then into subgroups of 10 rats for administration of bromopride or saline. Left colonic anastomosis was performed and abdominal sepsis was induced by cecal ligation and puncture. A colonic segment containing the anastomosis was removed for analysis of gene expression of MMP-1α, MMP-8, MMP-13, IL-β, IL-6, IL-10, tumor necrosis factor-α (TNF-α), and interferon-γ (IFN-γ). On the third POD, bromopride was associated with increased MMP-1α, MMP-13, IL-6, IFN-γ, and IL-10 gene expression. On the seventh POD, all MMP transcripts became negatively modulated and all IL transcripts became positively modulated. In the presence of sepsis, bromopride administration increased MMP-8 and IFN-γ gene expression and decreased MMP-1, TNF-α, IL-6, and IL-10 gene expression on the third POD. On the seventh POD, we observed increased expression of MMP-13 and all cytokines, except for TNF-α. In conclusion, bromopride interferes with MMP and IL gene expression during anastomotic healing. Further studies are needed to correlate these changes with the healing process.

PURPOSE: To evaluate the effects of copaiba oil administered by different routes on survival of mices subjected to cecal ligation and puncture. METHODS: Thirty two mice were distributed into four study groups (N=8): Sham group: normal standard animals; Control group: submitted a cecal ligation and puncture (CLP); Gavage group: submitted a CLP, and treat with copaiba oil by gavage; and Subcutaneous group: submitted a CLP, and treat with copaiba oil by subcutaneous injection. After the death of the histological analysis were performed. The Kaplan-Meier curves of surviving time were realized. RESULTS: All animals that received copaiba, regardless of the route used, survived longer when compared to the control group (p<0.0001), whereas the survival time ranged from 20 hours for the control group up to 32 hours for the animals of gavage group and 52 for subcutaneous group. The animals that received gavage copaiba lived about and about 20 hours unless the subcutaneous group (p=0.0042). There was no statistical difference when compared the intensity of inflammatory response (p>0.05) CONCLUSION: Prophylactic subcutaneous administration of copaiba in mice subjected to severe sepsis by cecal ligation and puncture, resulted in a survival time higher than non-use or use of this oil by gavage. .

BACKGROUND: Sepsis is a systemic inflammation associated with infection caused by pathogenic micro-organisms with high mortality rates. OBJECTIVE: In this study, we investigated the protective effect of Propionibacterium acnes-killed against polymicrobial sepsis induced by cecal ligation and puncture. METHODS: The mice were treated by intramuscular route in 1, 3, 5, and 7 days before the cecal ligation and puncture induction. The control group animals received vehicle (saline solution 0.9%) and the animals of the treated group received the P. acnes-killed (0.4 mg/animal). After anesthesia, midline laparotomy was performed with exposure of cecum followed by ligature and one transverse perforation of the same, with a 18 G needle, for induction of lethal sepsis. After surgery, the cecum of the animals was replaced into the peritoneal cavity, and it was closed with a 4.0 nylon suture. The survival of animals subjected to lethal sepsis was evaluated after cecal ligation and puncture induction. Six hours after the induction of sepsis, neutrophil migration, the number of bacteria, TNF-α, MCP-1, IL-6, and IL-10 were performed in the peritoneal lavage. RESULTS: Prophylactic treatment with P. acnes-killed increased the survival of the animals, followed by a significant decrease in the TNF-α, IL-10, and MCP-1 levels, 6 h after cecal ligation and puncture. Furthermore, P. acnes-killed administration reduced the number of bacteria in the peritoneal cavity with increased migration of leukocytes, especially neutrophils. CONCLUSION: P. acnes-killed promoted increased survival rate of animals with sepsis, in part attributed to its immunomodulatory properties against pathogenic microorganisms, as well as better control of infection by reducing bacterial counts.

ABSTRACT

PURPOSE: To compare sutures with polypropylene and poliglecaprone 25 after partial cecotomy in rats. METHODS: Thirty six rats divided into two groups, A and B, of 18 animals; each group was also divided into three subgroups of six animals sacrificed at 4th, 7th and 14th days after surgery. Were studied the mortality, morbidity, complications attributable to sutures, macroscopy, optical microscopy and measurement of hydroxyproline at the level of the suture. RESULTS: There were no deaths or wound complications such as hematoma, seroma, abscess, evisceration or eventration. On microscopic evaluation reepithelization, coaptation and inflammation in both groups did not differ significantly. The average rate of tissue hydroxyproline found in the samples on the 4th day after surgery, was 21.38 mg/g tissue for group A and 16.68 mg/g for group B; on day 7 after surgery, the average was 15.64 mg/g tissue for group A and 26.53 mg/g for group B; on day 14, the average was 8.09 mg/g tissue for group A and 25.07 mg/g for group B. CONCLUSION: There were no differences on clinical evolution, macroscopic aspect, microscopic data and hydroxyproline concentration on both sutures.


OBJETIVO: Comparar a sutura com fio de polipropileno e poliglecaprone 25 após cecotomia parcial em ratos. MÉTODOS: Trinta e seis ratos foram distribuídos em dois grupos A e B de 18 animais, e cada grupo foi dividido em três subgrupos de seis, sacrificados no 4º, 7º e 14º dias do pós-operatório. Estudou-se a mortalidade, morbidade, complicações atribuíveis às suturas, macroscopia, microscopia ótica e dosagem de hidroxiprolina no nível da sutura. RESULTADOS: Não houve mortalidade ou complicações da ferida operatória como hematoma, seroma, abscesso, evisceração ou eventração. Na avaliação microscópica os critérios de re-epitelização, coaptação e processo inflamatório ambos os grupos não apresentaram diferença significativa. A taxa tecidual média da hidroxiprolina encontrada nas amostras no 4º dia de pós-operatório foi de 21,38 mg/g de tecido para o grupo A e 16,68 mg/g para o grupo B; no 7º dia a média foi de 15,64 mg/g de tecido para o grupo A e 26,53 mg/g para o grupo B; no 14º dia ela foi de 8,09 mg/g de tecido para o grupo A e 25,07 mg/g para o grupo B. CONCLUSÃO: Não houve diferença estatística entre a evolução clínica, avaliação macroscópica, microscopia e dosagem de hidroxiprolina entre as suturas realizadas com os fios estudados.

ABSTRACT

Epidermoid cyst is a sac with benign characteristic that originates from embrionary ectoderm. Histologically consists in a thin layer of squamous epithelium. Epidermoid cyst of the cecum are exceptionally rare. We report a case of an epidermoid cyst arising in the anterior wall of the cecum, in a woman with no previous history of surgery, and review of the literature are presented.


Quiste epidermoide es un saco con características benignas que se origina del ectodermo embrionario. Histológicamente consiste en una delgada capa de epitelio escamoso. Quistes epidermoide de ciego son excepcionalmente raros. Reportamos un caso de quiste epidermoide en pared anterior del ciego en una paciente sin cirugía previa y realizamos una revisión de lo publicado sobre esta patología.

ABSTRACT

OBJETIVO: Avaliar a utilização do etil-2-cianoacrilato no tratamento de uma lesão provocada em um segmento parcialmente excluso do intestino do rato: o ceco. MÉTODOS: Utilizaram-se 45 ratos machos da linhagem Wistar, distribuidos em três grupos iguais, sendo que foi realizada a ressecção parcial do ceco. Os grupos foram denominados como: Grupo 1: a lesão foi tratada com aplicação de etil-2-cianoacrilato; Grupo 2: sutura e aplicação de etil-2-cianoacrilato; Grupo 3: sutura em bolsa. Os animais foram acompanhados no pós-operatório e metade de cada grupo foi necropsiada no 14º e restante no 28º pós-operatório. Dessa forma, foram submetidos à avaliação macroscópica, sendo coletadas amostras do ceco para avaliação histológica e, por fim, realizou-se a análise estatística RESULTADOS: O ganho de peso pós-experimento foi diferente nos grupos (p=0,028). A presença de microabcessos foi maior no 28º dia de pós-operatório no grupo 2, em comparação com o grupo 3 (p=0,003). A deposição de colágeno no 28º dia de pós-operatório foi maior no grupo 1 (p=0,036) e a intensidade da inflamação no 14º dia de pós-operatório foi maior no grupo 1 (p=0,045). Nos demais parâmetros avaliados, não ocorreu diferença estatística. CONCLUSÃO: A utilização do etil-2-cianoacrilato foi efetiva no tratamento do coto cecal excluso de ratos frente à avaliação macroscópica, microscópica e evolução pós-operatória.


OBJECTIVE: To evaluate the use of ethyl-2-cyanoacrylate in the treatment of an injury caused in a partially excluded segment of the mouse gut: the cecum. METHODS: We used 45 male Wistar rats, divided into three equal groups; in all there was performed a partial resection of the cecum. The groups were designated as Group 1: the lesion was treated with application of ethyl-2-cyanoacrylate, Group 2: suture and application of ethyl-2-cyanoacrylate, Group 3: purse-string suture. The animals were monitored postoperatively and half of each group was necropsied 14 days after the procedure, the remaining on the 28th. They were subjected to macroscopic evaluation, had cecal samples collected for histological examination and the findings were submitted to statistical analysis. RESULTS: Weight gain after the experiment was different among groups (p=0.028). The presence of microabscesses was higher at 28 days postoperatively in group 2 when compared to group 3 (p=0.003). The collagen deposition on the 28th postoperative day was greater in group 1 (p=0.036) and intensity of inflammation at the 14th postoperative day was greater in group 1 (p=0.045). In the other parameters there was no statistical difference. CONCLUSION: The use of ethyl-2-cyanoacrylate was effective in the treatment of cecal stump exclusion of rats as for macroscopic and microscopic findings and postoperative outcome.

ABSTRACT

PURPOSE: To analyze the effects of hyperbaric oxygen therapy (HBO) on the mechanical resistance of anastomosis performed in rats' distal colon presenting peritonitis induced by ligature and cecal punction using the Total Energy Rupture biomechanical test (ETR). METHODS: It was used 45 rats divided into three groups of 15 animals each. In Control Group (CG), it was performed anastomosis in distal colon without peritonitis. In Peritonitis Group (PG), it was performed anastomosis six hours after the induction of peritonitis by ligature and cecal punction. In Hyperbaric Chamber Group (HCG), it was performed six hours after the induction of peritonitis by ligature and cecal punction. The animals on CG and PG were kept at place air while the animals on HCG were placed on an experimental hyperbaric chamber in order to inhale oxygen at 100 percent, two absolute atmospheres, for 120 minutes, for four consecutive days. Euthanasia took place on the fifth day of the experiment. All the animals underwent to Total Energy Rupture Biomechanical Resistance test (ETR). Total Energy Rupture was defined as the necessary internal energy stored up to promote the colon rupture after an external traction force imposition. RESULTS: The Peritonitis Group presents smaller average ETR than Control Group. There was no statistical difference between Peritonitis Group and Hyberbaric Chamber Group. CONCLUSION: Hyperbaric oxygen therapy did not alter the mechanical resistance of anastomosis performed in distal colon of rats under the presence of peritonitis induced by ligature and cecal puncture.


OBJETIVO: Analisar o efeito da oxigenoterapia hiperbárica (HBO) sobre a resistência mecânica de anastomoses realizadas em cólon distal de ratos na presença de peritonite induzida por ligadura e punção cecal utilizando o teste biomecânico de Energia Total de Ruptura (ETR). MÉTODOS: Foram utilizados 45 ratos distribuídos em três grupos de 15 animais. No Grupo Controle (GC), realizou-se anastomose no cólon distal sem peritonite. No Grupo Peritonite (GP), realizou-se anastomose seis horas após a indução da peritonite por ligadura e punção cecal. No Grupo Câmara Hiperbárica (GCH), realizou-se anastomose seis horas após a indução da peritonite por ligadura e punção cecal. Os animais dos GC e GP foram mantidos em ar ambiente. Os animais do GCH foram colocados em uma câmara hiperbárica experimental para inalarem oxigênio a 100 por cento, a duas atmosferas absolutas, durante 120 minutos, por quatro dias consecutivos. A eutanásia ocorreu no quinto dia do experimento. Todos os animais foram submetidos ao Teste de Resistência Biomecânico Energia Total de Ruptura (ETR). A Energia Total de Ruptura foi definida como a energia interna acumulada necessária para promover o rompimento do cólon após a imposição de uma força externa de tração. RESULTADOS: O Grupo Peritonite apresentou menor média de ETR que o Grupo Controle. Não houve diferença estatística entre o Grupo Peritonite e o Grupo Câmara Hiperbárica. CONCLUSÃO: A oxigenoterapia hiperbárica não alterou a resistência mecânica de anastomoses realizadas no cólon distal de ratos na presença de peritonite induzida por ligadura e punção cecal.

INTRODUCTION: The antibacterial effect of ozone (O3) has been described in the extant literature, but the role of O3 therapy in the treatment of certain types of infection remains controversial. OBJECTIVES: To evaluate the effect of intraperitoneal (i.p.) O3 application in a cecal ligation/puncture rat model on interleukins (IL-6, IL-10) and cytokine-induced neutrophil chemoattractant (CINC)-1 serum levels, acute lung injury and survival rates. METHODS: Four animal groups were used for the study: a) the SHAM group underwent laparotomy; b) the cecal ligation/puncture group underwent cecal ligation/puncture procedures; and c) the CLP+O2 and CLP+O3 groups underwent CLP+ corresponding gas mixture infusions (i.p.) throughout the observation period. IL-6, CINC-1 and IL-10 concentrations were determined by enzyme-linked immunosorbent assay (ELISA). Acute lung injury was evaluated with the Evans blue dye lung leakage method and by lung histology. P<0.05 was considered significant. RESULTS: CINC-1 was at the lowest level in the SHAM group and was lower for the CLP+O3 group vs. the CLP+O2 group and the cecal ligation/puncture group. IL-10 was lower for the SHAM group vs. the other three groups, which were similar compared to each other. IL-6 was lower for the SHAM group vs. all other groups, was lower for the CLP+O3 or CLP+O2 group vs. the cecal ligation/puncture group, and was similar for the CLP+O3 group vs. the CLP+O2 group. The lung histology score was lower for the SHAM group vs. the other groups. The Evans blue dye result was lower for the CLP+O3 group vs. the CLP+O2 group and the cecal ligation/puncture group but similar to that of the SHAM group. The survival rate for the CLP+O3 group was lower than for the SHAM group and similar to that for the other 2 groups (CLP and CLP+O2). ABSTRACT

A úlcera solitária benigna do ceco (USBC) é uma doença rara que normalmente se apresenta como uma massa abdominal ou por sangramento digestivo baixo. O diagnóstico definitivo se dá pelo exame anatomopatológico da peça cirúrgica. Os autores reportam caso de paciente jovem que procurou a Unidade de Pronto-Socorro do Hospital Unimar - Marília com quadro clínico, exame físico e testes laboratoriais compatíveis com o diagnóstico de apendicite aguda. Foi indicada laparotomia com abordagem abdominal pela incisão de McBurney. Durante inventário da cavidade peritoneal, foi constatada massa palpável em região cecal e apêndice normal. Uma vez que não se conseguiu afastar uma neoplasia maligna do ceco, optou-se pela hemicolectomia direita seguindo preceitos oncologicos os, O diagnóstico de USBC se deu pelo exame histológico do espécime cirúrgico. Assim como descrito neste caso clínico, usualmente o diagnóstico de USBC se dá pelo exame histológico. Além disso, cerca de 60% a 80% dos pacientes se apresentam inicialmente com quadro clínico de apendicite aguda. O diagnóstico clínico da USBC por meio de uma colonoscopia é muito raro. Devido a isso, o tratamento, na grande maioria das vezes, é a hemicolectomia direita,uma vez que durante o intraoperatório não é possível afastar um câncer do ceco.

INTRODUÇÃO: O ozônio (O3) é encontrado na natureza e também pode ser produzido no corpo humano através da ativação de anticorpos. Seus efeitos anti-bactericidas são descritos na literatura, mas esses dados são controversos quanto a um potencial efeito benéfico da ozonioterapia no tratamento de certos tipos de infecção. OBJETIVO: Avaliar os efeitos da aplicação intraperitoneal (i.p.) de uma mistura gasosa de ozônio em um modelo de ligadura e punção de ceco (LPC) em ratos, através da dosagem de interleucinas (IL)-6, IL-10 e da quimiocina CINC-1 (cytokine-induced neutrophil chemoattractant), da lesão pulmonar aguda (LPA) e da análise das taxas de sobrevida. MÉTODO: Quatro grupos de ratos Wistar foram utilizados para análise de cada objetivo (CTR, LPC, LPC+O2 e LPC+O3). Os animais do grupo CTR foram submetidos somente a laparotomia. O grupo LPC foi submetido aos procedimentos de LPC. Os outros grupos foram submetidos à LPC e receberam injeção (i.p.) da mistura gasosa correspondente, administrada a cada 12 horas durante o período de observação. Os níveis séricos de IL-6, CINC-1 e IL-10 foram determinados por imuno- ensaio (enzyme linked immunosorbent assay- ELISA). A LPA foi avaliada através da histologia pulmonar e quantificada através do método do extravasamento pulmonar do Azul de Evans. Os animais da análise de sobrevivência foram observados por cinco dias. Os valores obtidos foram expressos como médias ± erro-padrão da média (EP) ou medianas mais percentis 25 e 75(P25; P75), de acordo com a distribuição dos dados.

BACKGROUND: Abdominal surgery can lead to postoperative intra-abdominal adhesions (PIAAs) with significant morbidity and mortality. This study compares the use of honey with a standard bioresorbable membrane (Seprafilm tm) to prevent the formation of PIAAs in rats. METHODS: Thirty rats underwent laparotomy, and PIAAs were induced by scraping the cecum. The animals were divided into three groups, each containing ten rats. Group 1 (control) represented the cecal abrasion group, with no intraperitoneal administration of any substance. Group 2 (honey group) underwent cecal abrasion and intraperitoneal administration of honey. Group 3 (Seprafilm tm group) underwent cecal abrasion and intraperitoneal Seprafilm tm application. RESULTS: Group 1 exhibited higher adhesion scores for adhesions between the abdominal wall and the organs. Groups 2 and 3 had decreased adhesive attachments to the intra-abdominal structures. Compared to group 1, the incidence of adhesion formation was lower in both group 2 (p=0.001) and group 3 (p=0.001). The incidence of fibrosis was also lower in group 2 (p=0.016) and group 3 (p=0.063) compared to group 1. There was no significant difference between the histopathological fibrosis scores for the rats in group 2 and those in group 3 (p= 0.688). CONCLUSION: This study suggests that both honey and Seprafilm tm decrease the incidence of PIAAs in the rat cecal abrasion model. Although the mechanism of action is not clear, intraperitoneal administration of honey reduced PIAAs. The outcome of this study demonstrates that honey is as effective as Seprafilm tm in preventing PIAAs.

ABSTRACT

Cecal Volvulus is an acute surgical emergency. In this condition cecum, terminal ileum and ascending colon on its mesenteric anticlock wise direction from 180 to 360 degrees. The cecal volvulus is caused by a congenital lack of fixation of cecum to retroperitoneum. The usual presentation consists of severe colicky abdominal pain with asymmetric abdominal distension. In this article two cases of cecal volvulus are defined. The first case was a 74 year-old female with sudden abdominal pain and severe abdominal distension subsequent 3 days patient often refers with sever colicky pain and asymmetric abdominal distention. After plain abdominal x-ray and abdominal CT scan, right hemicolectomy was performed. The second case was a 70 year- old female who subsequent 4 days without defecation was affected abdominal pain and distension. Supine and erect abdominal x-ray showed colon hyperairation and air-fluid levels. The gangrenous colon was resected etiology of bowel. Despite of other of obstruction or sigmoid volvulus that sometimes can be treated medically, the treatment of cecal volvulus is always surgery and consisted of cecum and ascending colon resection

ABSTRACT

PURPOSE: To evaluate the efficacy of inducing peritonitis in rats through laparoscopic cecal ligation (CL), by means of an elastic band. METHODS: Twelve Wistar rats were subjected to cecal ligated with an elastic band applied using a specially constructed applicator. In six of the animals (the CL group) the cecal sac was preserved intact whilst in the remaining animals (the CLP group) the sac was perforated with scissors. Clinical parameters, characteristics of the peritoneal cavity and cecum, and histological features of the cecal tissue were observed in all experimental animals 8 and 24 h after surgery. RESULTS: CLP animals exhibited at least one clinical sign of sepsis within the first 8 h of observation. The peritoneal liquid was observed to be clear in almost all members of the CLP. Polymorphonucleated cells were detected in the tunica serosa of the cecum of CLP animals. In contrast, all members of the CL group were alive after 24h, and of polymorphonucleated cells in the muscle layer of the cecal wall were observed. The presence of peritoneal liquid was not detected in CL animals. CONCLUSION: Although elastic ligation of the cecum was reproducible, puncture of the cecal sac was essential for induction of sepsis.


OBJETIVO: Avaliar a eficácia de induzir peritonite em ratos através da ligadura cecal laparoscópica (CL), por meio de banda elástica. MÉTODOS: Doze ratos Wistar foram submetidos a ligadura cecal com banda elástica aplicada com dispositivo especialmente construído para este fim. Em seis animais (grupo CL), a bolsa cecal foi mantida intacta, enquanto nos outros animais (grupo CLP), a bolsa foi perfurada com tesoura. Parâmetros clínicos, características da cavidade peritonial e ceco, e histologia do tecido cecal foram examinados em todos os animais 8 e 24 horas após a cirurgia. RESULTADOS: Os animais do grupo CLP apresentaram pelo menos um sinal clínico de sepses nas primeiras 8 h de observação. Líquido peritonial claro foi observado em quase todos os membros do grupo CLP. Leucócitos polimorfonucleares foram identificados na serosa do ceco dos animais do grupo CLP. Por outro lado, todos os animais do grupo CL estavam vivos após 24 h, e leucócitos polimorfonucleares estavam restritos à muscular própria. Presença de líquido peritonial não foi detectada nos animais do grupo CL. CONCLUSÃO: A ligadura elástica do ceco foi reprodutível e a secção da bolsa cecal foi essencial para a indução de sepse.
